We help people overcome drug problems, other health conditions, and arrest histories. Through our advocacy and case management services, we show our clients that their past isn’t necessarily the path of their future, and that they can succeed in changing their lives.

Throughout Illinois, we work with men, women, and youth who are involved in courts, prisons, public health, and child welfare systems. We connect these government systems to services in the community.

Success through Case Management

TASC does not provide treatment, but instead we are independent advocates who place clients into community-based treatment programs and other services that will help them succeed. We also provide ongoing case management, often for many months or more.

People who receive TASC’s case management services are more successful—in terms of reduced drug use and reduced use of government programs—than those who don’t get TASC.

Our goal is to help adults and youth become healthy and responsible citizens and family members. Through our statewide team of care coordinators, we help our clients live up to the hopes they have for themselves and the promises they make to others.

Statewide Services and National Consulting

There are a number of TASC programs throughout the United States. The TASC model was introduced to Illinois in 1976, and we now reach nearly 29,000 people across the state annually.

Our partners include more than 240 community-based treatment providers across Illinois, along with numerous supporters at local, state, and national levels.

Also, through our Center for Health and Justice, we offer consulting, training, and broad-based public policy solutions nationally and internationally.

YSP provides positive alternatives for youth in the neighborhood. Through a spectrum of enrichment opportunities, YSP meets youth’s needs while focusing on young people’s interests, assets and strengths. YSP works with youth rather than for youth, through program offerings in seven core areas: education, recreation, intervention, prevention, arts and culture, community building, and diversion.

Through our services, YSP works with youth and their families to overcome difficulties and stabilize lives. Our programs, focusing on community building, engage youth, their families and the community at large in activities that promote positive youth development.

Current programs:

“To The Moon” Art Program
An arts and culture education program that teaches the value and different mediums of arts as well as the importance of cultural identity and diversity.

Addiction Intervention Program
A harm-reduction program designed to help youth address issues related to triggers leading to social and legal problems stemming from drug and alcohol use/abuse.

Transitional Living Program
A comprehensive case management and housing program for female youth, ages 17½ – 21, who are in DCFS state care and are preparing to live independently after they emancipate from Illinois’ child welfare system.

Independent Living Program
A comprehensive case management and housing program for youth in DCFS state care, ages 19 – 21, who are preparing to live independently and have obtained their high school diploma, or its equivalent, and are working or attending higher education.

Crisis Intervention Program
A service for youth displaced from their homes due to family and legal crisis or conflicts, designed to stabilize, intervene, and enhance the family unit.

Clinical Services
Supportive and culturally-responsive individual, family, and group counseling utilizing art, talk, and behavioral therapies to assist youth in enhancing their strengths.

Youth Leadership Academy Program
A community-based youth-led leadership training and development program for Humboldt Park youth, ages 14 – 24.

Career & Workforce Development Program
Various career readiness programs for youth, ages 11 -24. Youth will have an opportunity to complete a part-time internship in various sectors of their career interest, explore careers, job readiness, and venture on high school, college, and trade school visits.

Anti-Bullying Program
A school-based program for middle school-aged youth, their parents, and teachers that aims to increase knowledge and provide strategies to decrease bullying behaviors, empower bystanders, and embolden victims to respond nonviolently to bullying.

Juvenile Justice Community Support Program
An intensive case management program for justice-involved youth, ages 12 – 18. Our model works closely with families and communities providing creative, empowering, and culturally rich services to disconnected and vulnerable youth.

San Lucas United Church of Christ is a member of the United Church of Christ, a Christian denomination made up of 5,600 churches and 1.2 million members.  Reverend Jorge Morales founded San Lucas Church in 1976.  The church’s original founding members was composed of thirty Christian conscious men and women, who were raised in the Humboldt Park community, and sought a Christian remedy to the ills of poverty and deprivation faced by the community at large.  They sought to make concrete changes in the day-to-day lives of people living in the Humboldt Park and West Town community, which at that time was predominantly Puerto Rican.

Today, the members of San Lucas United Church of Christ are full of the love of God and the Grace of our Lord Jesus Christ, and have totally identify with the needs of an oppressed people that are in search of freedom.  This is why San Lucas Church is committed and is n solidarity with the struggle of the poor.  The church is committed with other institutions that struggle for justice, peace, and the liberation of people.  San Lucas Church motto is, “Faith In Action”.

San Lucas United Church of Christ provides worship services on Sunday at 11:00 a.m.  Other services provided by San Lucas Church include the Hot Meal Program, where every Tuesday at 12 Noon, community residents are provided with a hot meal; the Food Pantry provides community residents with groceries every 2^nd & 4^th Saturday of the month; the Thrift Store provides residents with clothing/shoes, household needs, on Monday thru Friday from 10:00 a.m. – 1:00 p.m.

In collaboration with the Chicago Food Depository, San Lucas Church provides fresh fruits and vegetables through the Produce Mobile Program, every Third Monday of the month; community residents are provided assistance with their energy bill during the winter months through the Low Income Home Energy Assistance Program, sponsored by the Community and Economic Development Association of Cook County, In.; after school program is provided to community youth through the Teen Reach After School Program through the BUILD Organization  – Monday thru Friday at 3:00 p.m.; weekly creative writing workshops sponsored by the Neighborhood Writing Alliance,  are held every Wednesday at 6:30 p.m.

In addition, over the last Five years, San Lucas Church, in collaboration with the Puerto Rican Cuatro Project, offers the Annual Puerto Rican Tiple Construction Workshop.  This workshop provides community residents/participants the rudiment of stringed instrument making, as well as to provide information about this extinct string instrument, its cultural history, its music, and place in Puerto Rican society.  The church has also provided its space to other community organizations to present their programs and events.
